Help! How do I write a contract for working with a self published author?

Hi everyone. I'm assuming that some of you at least have worked with self published authors, so I'm hoping someone can point me towards some resources to use when writing a contract.
I've been asked by a self publishing author to do a cover for her book, as well as some chapter heading illustrations. She has asked if I would use a work for hire contract, and I already know that I don't want to do that. What I want to do is license my artwork for her for the first edition of her book, as well as for the ebook. But I'm at a loss when it comes to actually writing that out in contract language! Do any of you know of resources, contract templates, etc to help illustrators in this situation?

I had a zoom interview with a self published author a couple days ago (turned out the author didn't do any research, had no business plan, or concept of budget, I turned this project down for this reason) and so, when researching contracts with self published authors I found this resource, "Business and Legal Forms" by Tad Crawford, and he has many other variations in this series, including one for Illustrators. And so in this I imagine there will be many iterations of legal forms that might be useful. Worth checking into?

This article from the Business of Illustration website was also very helpful to me when I was drawing up my own contract template for illustration jobs.
